Need to make survey and don't want to end up with a million emailed responses? Use Google Forms to make a survey to gather data and information. You can add questions with a variety of ways for respondents to put in information. Those ways include multiple choice, drop down, grids, scales, date, time, short or long responses.
Even more exciting, you can create forms that have YouTube videos and/or images. Videos can appear before or after questions, while images can be used as part of a response/answer to a question.
